I will be 18 years of age or older on the date of the session(s) I am attending.

I understand that Freestyle Dance Alchemy classes are non-refundable and non-transferrable. Purchases can only be used during agreed upon validity periods.

I understand that start and end times are fixed. If a participant needs to leave early or doesn't show up, there will be no refunds issued. If a participant arrives more than ten minutes late, the door may be locked and they will not be able to join the session, but a credit can be issued for use toward the next session upon request. 

I understand that my Freestyle Dance Alchemy facilitator is not a clinician and cannot treat, diagnose, or cure any disease, illness or injury. In all cases, participants should join classes with approval from a trusted medical professional and should always follow their doctor’s advice.

I take full responsibility for my own health and any illness or injury that may occur. I cannot and will not hold my facilitator or any employers or affiliates responsible in the case of accident or illness during practice.

I will notify my facilitator before the session begins if I have any health or movement challenges she should know about including accommodations eg. chair to use in lieu of sitting on the floor.

I give permission for moments of FDAW to be filmed or photographed knowing that my identity will be de-emphasized or hidden in any promotional material where such material is used, and I take responsibility for notifying my facilitator if I would like to withdraw my permission.

I understand that the facilitator determines what belongs in the session or circle. As a participant, I agree to trust this guidance.

I am providing my contact information for class details, payment links, updates, invitations to future dance sessions, and occasional marketing communication. I can be removed from the list at any time by contacting my facilitator.

I have read and understand the terms and conditions of Freestyle Dance Alchemy class as outlined above and in descriptions provided by Humanist Coaching and/or affiliate hosts.
